Member using ladder, falling from ladder due to ladder being moved whilst in use

Members using ladders to have a second person footing the ladder unless it is secured. Members to keep 3 points of contact on ladder at all times. Ladders to be correctly sited, and a watch officer to be appointed to ensure ladder remains correctly positioned and is not at risk of movement.
